People with developmental disabilities often need assistance and supports to live their daily lives.    These supports could include a mentor who helps you with activities of daily living, learning new skills, working in the community or creating social connections.   New York State offers people the opportunity to to self direct and control their services.  Self Direction provides flexibility and allows individuals to adapt or change goals, schedules, activities, and expectations based upon the needs of the individual.

 Self Direction begins with a person-centered plan that outlines your goals and objectives.    It includes a specific support plan that describes direct staff supports, mileage, and other services that you may need.     Self Direction allows you to manage these supports and services yourself with the help of Advocates.   You have full budget and employer authority and together we help you lead a fully inclusive life in your community. 

 

Advocates will help you plan for your services through a process called Person-Centered Planning. This is an individualized approach to planning that focuses on the individual's strengths and capabilities while addressing their needs.  We will work with you and your family to manage your customized plan on an on-going basis. It is a collaborative model whereby you, your family and your Circle of Support will work as a team with Advocates to schedule, hire, train and supervise your staff. Advocates will help you to identify resources, schedule shared/split staff, find community integration opportunities, network with peers and help you manage all of the paperwork.
